I am the parent or legal guardian of the youth named above, or I am otherwise legally authorized to give this consent. I give permission for the youth to participate in the event or activity identified above, including reasonable related transportation when arranged by Gravel Hill United Methodist Church. I confirm that the information on this form is accurate and that I have disclosed conditions or limitations relevant to safe participation.
If I cannot be reached promptly in an emergency, I authorize Gravel Hill staff, volunteers, event personnel, and licensed medical providers to obtain or provide reasonably necessary first aid, emergency treatment, medication, transportation, hospitalization, anesthesia, surgery, or other care for the youth. I understand that reasonable efforts will be made to contact me and that I am responsible for costs not covered by insurance.
I understand that youth activities may involve risks, including travel, games, sports, outdoor activities, food, equipment, slips or falls, contact with other participants, illness, and other foreseeable or unforeseeable hazards. I knowingly allow the youth to participate and accept the ordinary and inherent risks of the activity.
To the fullest extent permitted by Pennsylvania law, I release and hold harmless Gravel Hill United Methodist Church, its clergy, employees, officers, members, volunteers, agents, and event partners from claims arising from the youth's participation or related transportation, except to the extent a claim results from conduct that cannot legally be released, including gross negligence, reckless conduct, or intentional misconduct. This section does not waive any right that Pennsylvania law does not allow a parent or guardian to waive on behalf of a minor.
The youth may participate regardless of the selection. Gravel Hill will not intentionally publish the youth's full name with an image unless separate permission is obtained. I authorize Gravel Hill United Methodist Church to photograph, video, or audio record the youth during church activities and to use that media, without compensation, in church publications, worship presentations, the church website or app, email, and official social media for ministry, informational, or promotional purposes.
I understand that participants are expected to follow safety instructions and conduct standards. I authorize Gravel Hill to contact me if a health, safety, or behavior concern arises and understand that I may be asked to arrange the youth's early pickup when necessary.
I have read and understand this form, had the opportunity to ask questions, and voluntarily agree to its terms. Unless a shorter event period is written above, this form applies only to the identified event or season and may be revoked prospectively in writing, except for actions already taken in reliance on the permission granted.
